Safety and escalation
The hard rules: safety sequences verbatim and first; bypass requests refused; uncovered symptoms and hybrid units escalate with evidence.
Safety sequences
Marked during index review (lockout/tagout, isolation, pressure release, PPE, confined-space). In every checklist they appear first, reproduced from the manual in sequence, and cannot be collapsed, reordered or omitted by the technician. Bulletin additions are merged in.
Bypass requests
Any request to defeat an interlock, guard, limit or safety function — from a technician or relayed from a customer — is refused. The response cites the manual's position and gives the escalation path. There is no configuration that changes this behaviour.
Escalation triggers
- Symptom not covered by any manual, bulletin or indexed work order
- Hybrid unit (subsystems at different revisions) unless the product line has approved composite procedures
- Conflicting instructions between manual and bulletin that the index could not resolve
- Technician reports a measurement outside any branch of the tree
- Safety-adjacent judgement calls
Escalation package
Symptom and alarms; unit identification and history; photos; checks performed and results; what the documents cover and do not; prior similar work orders. Addressed to the named senior engineer for the product line; logged.
From escalation to bulletin
When the same uncovered symptom is resolved by an expert three or more times, a draft bulletin is generated from their recorded resolutions (symptom, root cause, procedure, source work orders) for the expert's approval. Approved bulletins enter the index with the approver's name and date.
Reporting
Escalations by model, symptom, dealer and outcome, for documentation planning and training.
